How to Ask My Insurance for Counseling at The Space Between
Call Member Services using the number on your insurance card, use video relay, or send a secure message through your plan’s portal. Choose the wording that matches your needs.
START WITH THIS REQUEST
“I would like to receive counseling at The Space Between. Please check whether the practice and the counselor I would see are in network for my specific plan. What do I need to do to receive covered services there?”
If you want to request Joy specifically, add: “I would like to see Joy Plote, LPC, if she is available.”
IF YOU NEED COUNSELING IN ASL
“I am Deaf and use ASL. I am requesting counseling directly in ASL with a clinician who understands Deaf culture and my treatment needs. I would like to receive this care at The Space Between.”
IF YOU NEED A COUNSELOR FAMILIAR WITH CODA EXPERIENCES
“I am a Coda—a hearing person with Deaf parent(s). My family’s language and culture are important to the concerns bringing me to counseling. I would like a counselor at The Space Between who understands these experiences.”
IF THE COUNSELOR IS OUT OF NETWORK
“Please review whether your network has an available counselor who can meet my language, cultural, and treatment needs. If it does not, please review a network gap exception so I can receive covered care at The Space Between at the in-network benefit level. Please contact the practice about any needed authorization or single-case agreement.”
BEFORE YOU FINISH
“What is the next step? Do you need information from me or the practice? Please give me a reference number and send the decision in writing, including any approved services and my share of the cost. If you deny my request, please explain the reason and how and when I can appeal.”
Keep the date, representative’s name, reference number, and next step. Share the reference number and written decision with The Space Between so the office can verify coverage and payment arrangements. A request does not guarantee approval.
